HEREBY VOLUNTARILY GIVE, DONATE AND PERMANENTLY RELEASE to (Rescue), the

interest in the animal/s and that I understand that I am releasing,
discharging and agreeing to hold harmless said (Rescue) from any liabilities

	I further attest that I am releasing this animal/s completely,
voluntarily and that no promises of any kind have been made to me by
(Rescue), or any of its agents.
