I have been putting together an 'initial contact form' for donated horses for our rescue. It has donors info (name, address, telephone, email, etc.) plus info on the horse. I have blanks for name, description, breed, ailments or medical conditions, lamenesses, vaccinations, coggins, last teeth floating and worming, etc. I also have blanks for what the horse currently eats and such. Some of this stuff might not get filled out but it gives me a list of questions to ask potential donors. It also has a section at the end for the volunteer doing this initial contact with the potential donor. It reminds them to let the donor know they give up all rights when they donate the horse, that we DO charge an adoption fee, and a few other items we need to tell donors. As far as potential adopters go, we have those interested in adopting fill out an application. I don't really keep a record of those asking for horses we don't have (so they don't fill out an app.). For adoption applications we charge members $5 and non members $15 (but if they like, they can put $10 of that $15 towards a membership).
